However, in booking an appointment our staff and surgeons may request and collect your personal information including, but not limited to, your name, date of birth, residential address and various forms of identification (Medicare card and health fund details). All personal information is kept confidential and secure in keeping with the standards and policies outlined in the Privacy Act.
